DROP

To Join DROP

For active members who want to join DROP, a DROP Election for Active Members Form will need to be completed. This form is currently being updated and is not available on this Website.Please call a Benefits Counselor at the Pension System to make an appointment to discuss joining DROP.

For Retirees in DROP

DROP Distribution and Deferral Election and Change Form (This form is used for four purposes: to take a lump sum distribution from DROP, to change the amount added to your monthly benefit from DROP, to defer money into DROP, or to change the amount that one defers into DROP.
 
 

Service Retirement

For active members who want to retire, please print and complete the forms below and read the information provided. Bring the completed forms to the Pension System office.

Member Information Consent form (The law protects your confidentiality as a Member of the Pension System. This form grants permission for the Pension System to share your name and photo in our communications (newsletters, Web, etc.).
Autopay Authorization (This form authorizes certain payments, such as insurance, from your bank account)
Direct Deposit Authorization form (This form is used to begin direct deposit or make changes to your direct deposit information)
 
 
2012 IRS Form W-4P Federal Income Tax Withholding form (This form is used to establish the amount withheld for Federal Income Tax)

Change of Address form for Pensioners (Active Members must change their address with their department and through the City of Dallas Lawson System)
